Please print these forms and complete them prior to your first appointment. The Health History form is quite lengthy, so please allow yourself enough time to answer all the questions before your appointment. Make sure you bring a list of all your current medications. If you think you have insurance coverage for acupuncture, please call your insurance company before your visit; use the "Questions for Insurance" document to make sure you get all the information I need to bill your insurance company.
